We are guided by the principle that, because Alzheimer’s and dementia affect every person differently, each resident must be cared for as an individual. Recognizing that there is no one-size-fits-all approach, we offer multiple levels of dementia care options, allowing you to select the most appropriate services for your loved one, and then change or add to those services over time.

We understand that this is a difficult time for your family and that selecting a dementia care assisted living community is an important decision. That is why we offer a deeply personal approach that values both you and your loved one as critical participants in the care process.

Our Approach to Residents

The Memory Care Program at New Horizons honors an individual’s story. All individuals are appreciated for who they are, for how they define their past and present, and for their current abilities and focus. Our program is one of acceptance, appreciation, and respect for each individual’s diversity, traditions, and personality. We focus on the emotional, physical, and environmental “positives” of our residents. We understand that each day is a new day and that abilities and interests change. It is not about having answers to every situation, but rather creating a sense of acceptance, non-judgment, and flow to the day. We believe each day should be pleasurable, inviting, active, and meaningful.

Our Approach to Family

The Memory Care Program at New Horizons values you as part of this journey. We encourage your calls, your visits, your insights, and your collaboration. We know this disease process is difficult on all involved, and we are here for you. Your thoughts, feedback, and observations are critical. Our commitment to you is to provide support and ongoing communication and to be there for you when you need us the most. Please join us for monthly support meetings to share your thoughts, connect with other families and loved ones, and gain support during the many transitions you and your loved ones are managing.

Memory Care Service Plan Options

Our Memory Care program offers multiple levels of specialized care, described below, based on your loved one’s needs. Under the supervision of a nurse, personal care assistants help residents in the privacy of their own homes. The majority of personal care is provided by Mary Ann Morse Healthcare, a Medicare-certified home health agency that maintains a 24-hour staffed office at New Horizons to provide the care described below:

Level One Plan:

  • Hourly overnight checks
  • Laundry service
  • Assistance with showers
  • Daily bed making and trash removal
  • Daily escorts to all three meals and activities
  • Daily AM scheduled assistance with grooming and dressing
  • Daily PM scheduled assistance with grooming and dressing
  • Up to two medication passes provided by a certified personal care attendant (SAMM) or limited medication administration (LMA) provided by a licensed nurse
  • Additional medication passes available for a fee

Level Two Plan:

  • Hourly overnight checks
  • Laundry service
  • Assistance with showers
  • Daily bed making and trash removal
  • Daily escorts to all three meals and activities
  • Daily AM scheduled assistance with grooming and dressing
  • Daily PM scheduled assistance with grooming and dressing
  • Up to two medication passes provided by a certified personal care attendant (SAMM) or limited medication administration (LMA) provided by a licensed nurse
  • Toileting assistance / Incontinence management every four hours

Level Three Plan:

  • Hourly overnight checks
  • Laundry service
  • Assistance with showers
  • Daily bed making and trash removal
  • Daily escorts to all three meals and one activity
  • Daily AM scheduled assistance with grooming and dressing
  • Daily PM scheduled assistance with grooming and dressing
  • Up to two medication passes provided by a certified personal care attendant (SAMM) or limited medication administration (LMA) provided by a licensed nurse
  • Toileting assistance / Incontinence management every two hours
  • Two-person assistance with tasks, as needed

A la Carte Services: Mary Ann Morse Healthcare may make additional, supplementary items and services available at its standard charge for the applicable items and services.

The services listed below are considered additional services. Regular use would require a service plan.

Service plans are tailored to meet the specific requirements of each resident, and are updated as the resident’s health and/or situation changes. Memory Care services fall under the guidance of the multi-disciplinary team for case and care management.

  • A review meeting is typically held 30 days after the start of the service plan to verify appropriateness.
  • Semiannual reviews are held thereafter to verify appropriateness of the resident’s service plan, in accordance with the regulations of the Executive Office of Elder Affairs.
  • The resident and/or resident’s family may request an ad hoc service plan review at any time by calling the Mary Ann Morse Healthcare program coordinator to schedule an appointment.
